Jockey Club Cancer Care for Survivors Project

With the generous support of The Hong Kong Jockey Club Charities Trust, the Hong Kong Anti-Cancer Society is honored to partner in the Jockey Club Cancer Care for Survivors Project.

This initiative is explicitly designed for adult cancer survivors who have completed their primary treatment within the last three years. Our objective is to cultivate a robust and sustainable support network within the community. Through comprehensive physical and psychological health management, we aim to seamlessly connect survivors with appropriate community resources, assist in mitigating the risk of recurrence, and empower them to regain confidence in leading a healthy life, fulfilling life.

Project Highlights
  • Clinical Leadership: Spearheaded by senior nursing professionals and supported by a multidisciplinary healthcare team, the project provides survivors with a robust foundation of clinical expertise.
  • Personalised Health Management: Utilising an extensive network of community resources, we develop bespoke health management plans for each participant, designed to comprehensively enhance their overall quality of life.
  • Fully Funded: All assessments and support services provided under this initiative are completely free of charge, thereby alleviating any financial burden on our survivors.

  • Symptom Management: Professional guidance and therapeutic strategies to alleviate physical discomfort arising from cancer or its associated treatments.
  • Weight and Nutrition Support: Assisting survivors in cultivating healthy dietary and lifestyle habits to optimize their physical function.
  • Psychological Wellbeing: Dedicated counseling and emotional support to accompany survivors throughout their recovery, fostering resilience and a positive outlook.

Service Pathway
  • Comprehensive Screening and Assessment: An in-depth evaluation of each survivor’s physical and psychological condition alongside their specific needs.
  • Development of a Tailored Care Plan: Formulating bespoke recovery and health management strategies based on individual assessment outcomes.
  • Integration with Community Services: Facilitating precise referrals to relevant community support frameworks, ensuring survivors receive long-term and continuous care.

Eligibility Criteria
  • Diagnosis: Previously diagnosed with Stage I to Stage III cancer.
  • Treatment Status: Completed primary cancer treatments (including surgical intervention, chemotherapy, or radiotherapy) within the preceding three years.
  • Age: Adult survivors aged 18 years and above.
